TO BE ELEGIBLE

  • Be 16 years of age or older.
  • Need assistance with activities of daily living provided by a live-in caregiver.
  • Be unable to live alone due to special needs related to mental health issues, disorders associated with aging, dementia and other memory impairments. evolving medical condition that prohibits them from independently completing their Activities of Daily Living.
  • Be eligible for MassHealth Standard or Common Health (in Massachusetts).
